Mainstream House Structure Agreement
1) No using illegal or illicit drugs,including alcohol.If you are on prescribed medication, staff must be informed.If you are prescribed medication at any time during your stay,staff must be informed.
2)You must be home for curfew,and in your house or apartment,by 11:30 p.m., Monday thru Thursday,and 1:30 a.m., Friday and Saturday(curfew your first two weeks is 11:30 p.m. every night except Sunday.)
3)You must attend a mandatory community meeting every Sunday@ 5:30  p.m.( no exceptions)
4) Rent is due 6:00p.m. every Friday.You must be there unless other arrangements have been made.
5)You must be out of the house/apartment by 9:00a.m. Monday thru Friday,either working or actively seeking employment.(Unless your are doing volunteer work etc.)
6) You must be home Sunday at 10:00 for a mandatory clean-up of your house/apartment.
7) You must be willing to submit to random drug testing.If it’s random,it’s MAINSTREAMS expense. If it’s suspected, it’s the clients expense. Cost---$25.00
8) You must have a daytime job within the first two weeks.
9) You must commit to a four month stay.
10) All parties involved in any physical confrontation, will be discharged.
11) No Stealing,this includes borrowing anything, without permission.
12) No overnite guests.All visitors must be out by curfew.
13) No one except you and your roommate are allowed in the bedrooms.
14) Absolutely NO  female guests allowed in,or hanging around living quarters.
15) No gambling of any kind.
16) Absolutely NO smoking in bedrooms.Please be considerate of others when smoking.Please smoke only in designated areas.
17) Your room and house/apartment must be clean at all times,and your bed MUST be made each morning.
18) Dishes must be cleaned IMMEDIATELY after use ,and then put away.
19) You must make 90 meetings in 90 days. Then,you MUST make 5 meetings each week from that point on.

20) You MUST get a Sponsor.

21) Absolutely no pornographic material allowed in the house.

 
22) The undersigned (licensee)understands,acknowledges and agrees that because of the obligation of the license or(Mainstream), in the necessity of maintaining a safe and orderly enviorment,any violation of this agreement and/or including a general bad attitude towards others is a substantial violation of this license agreement and automatically terminates licensee’s rights to occupy the house/apartment, and that may result in IMMEDIATE dismissal.
